Not sure how consistent this is but its happened to me about 4 times now. Last time I recorded the actual progress in the initialization process and it was between the output of "Using accel calibration" and the output for enabling interrupts (which is newish).
When it happens it is as though the processor freezes as the network goes down and the keyboard/video doesn't work either (and of course the program appears to not operate). It is particularly prickly if Qt Creator is running because one time it zero'd out a source file and I had to re-write it.

Happened again, this time with the controller all back together. Seemed to die at the same spot as last time. Might be in the RTIMULib functionality - hard to tell. Need to find out if I can enable some kind of crash diagnosis mode.

Happens less often now that I added a "wait for reset" cycle in the RTIMULib code (during initialization of the MPU9150 that is). I did witness one post edit crash at what appeared to be the same area of the code - but I have also seen more crashes completely independent of our running application.
